Title: Activity Aide

Supervisor Responsibilities: No

Location: 121 W. 154th St in Harvey, IL

Shift: 7a - 3:30pm

Purpose: The Activity Aide implements recreational and activity services in compliance with CHC policies and procedures.

Responsibilities:

• Implements activity and after school programs according to the written schedule
• Document progress and attendance for all activities (both group and individual)

• Completes documentation daily
• Assists with completion and implementation of documentation of habilitation plans
• Maintains records and prepares requested reports
• Complete activity notes by the 10th of each month
• Participates in the continuing interdisciplinary evaluation of individual residents needs

• Assures there is written permission given by the resident's physician to participate in the activity program
• Implements the activities to coordinate with other departments to ensure communication between all departments to provide uniformity of goals
• Participates in transporting students to and from the activities
• Assist in the development and implementation of special events
• Assist in the coordination of holiday visits from various groups/foundations

• Suggest ideas for in-house and community activities
• Attends seminars/workshops/ facility/department meetings as necessary

Qualifications:

• High School diploma or equivalent
• Experience working with students with complex medical needs and intellectual disabilities.

• Ability to pass criminal background check and drug testing
• Experience working with various socioeconomic populations is a plus.
• Must be able to lift, push, pull or carry a minimum of 50 lbs.
