POSITION SUMMARY: The Admissions Registered Nurse, will report to the Patient Care Manager. This position performs pre-admission evaluations for appropriateness for services. Initiates the admission process by educating and informing patient/family of its services. This position also obtains informed consents, provides nursing assessment, initial plan of care and intervention(s) that optimize patient and family directed comfort and quality of life.

HOURS: (3) 12-hour shifts/week 10AM-10PM - Plus Two Weekend Days/Month

SERVICE TERRITORY: Arlington Heights, Elk Grove Village, Barrington, Lake Zurich & surrounding areas.

POSITION REQUIREMENTS:

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of IL
• Must possess current CPR, First Aid Certification
• 1 year Hospice experience, preferred
• Must possess the ability to make independent decisions when circumstances warrant
• Must possess the ability to deal tactfully with patients, family members, visitors, agency personnel, and general public
• Must be knowledgeable of quality assessment and assurance procedures
