Working collaboratively with a physician and under the clinical supervision of the Medical Directors, the Advanced Practice Nurse (APN) provides medical evaluations and ongoing medication treatment to patients. The specific protocols of treatment are worked out by the APN and the collaborating physician through a practice agreement. The APN provides consultation to clinical staff regarding patient care issues, and may also provide formal in-service education and training to other employees. Participates in a collegial relationship with the physicians, nurses, and other health care professionals. Performs in an expanded nursing role and has responsibility for meeting the needs of a determined number of patients using a primary care model. The APN may work in multiple programs and different sites on different days. This position requires a high level of professional responsibility, medical judgment, and ability to take personal initiative while working as part of a team.
Among the key duties of this position are the following:
- Participates in data collection related to the Hospitalist patients’ status and needs.
- Provides direct patient care within scope of practice to ensure implementation of the plan of care.
- Informs and instructs patients/significant others on patient’s health/illness state and plan of care.
- Documents all aspects of patient care in EMR system.
- Is knowledgeable of and initiates emergency procedures.

- At least two (2) years of related nursing experience preferably as an advanced practice nurse
- Certified as either Clinical Nurse Specialist or a Nurse Practitioner in the State of New Jersey.
- Maintain current CPR for healthcare provider certification.
- Demonstrated experience in treating a diverse patient population.
- Bilingual skills (English/Spanish).
