In accordance with established procedures, abstracts clinical information from a variety of medical records to meet quality measures and benchmarks. Works as in integral member of the clinic team to identify opportunities for quality measure improvement.
Works to establish and maintain a robust quality program that promotes health and wellness to the employees and patients of Lane Regional Medical Center.

• Work with clinic leadership team to establish quarterly and annual goals for quality improvement measures.
• Educates providers and staff on quality metrics and processes for capturing metrics.
• Works with staff to close gaps in documentation to meet quality metrics both within and outside of hospital system.
• Seeks and attends seminars relevant to quality and patient outcomes.
• Builds strong working relationships with offsite facilities and community partners, establishing primary contact for each relationship to help successfully capture and record data.
• Maintains programs which capture quality data and benchmarks through EMR and third-party vendors.
• Monitor provider quality measure completion rates and quality documentation

o Provides guidance on measure specification and benchmarks across all patient types and payors to provider teams through regular meetings
Minimum Requirements
Education:
High School Diploma or equivalent.

• At least one year of medical experience preferred in quality, health coaching, or patient programs.
