Job Description

As part of NYC Health + Hospitals (H+H), the Office of Population Health (OPH) has a broad portfolio encompassing ambulatory care transformation, development of innovative care models, chronic disease prevention and management, research and evaluation, population health analytics, and social determinants of health (SDOH). The SDOH team sits within OPH and supports H+H facilities and community partners to routinely and effectively address patients’ social needs. Social needs are non-medical aspects of individual and family lives that can impact one’s ability to maintain health and well-being. The SDOH team focuses on social needs such as: housing (stability and quality); food (stability and security); legal needs (immigration status, eligibility for public health insurance, employment, family, and/or housing access); and government benefits access that relates to income maximization.

We are seeking an Assistant Director who will report to the Senior Director on the Social Determinants of Health team and focus primarily on building NYC H+H’s capacity to identify patients with food insecurity (e.g. lack of access to adequate food) and implementing strategies to meet those needs. Examples may include but not be limited to helping patients enroll in relevant public benefits, accessing new resources such as Medically Tailored Meals, or connecting with community-based resources such as culturally relevant emergency food.

General tasks and responsibilities will include:

  • Address patients’ food insecurity by coordinating day-to-day operations and workflows, for example by supporting access to Food Navigators, Medically-Tailored-Meals (MTM), and Groceries to Go (G2G)
  • Provide guidance to contracted staff, partners, and H+H hospital and clinic staff as their work relates to addressing food insecurity, including but not limited to building strong relationships, leading meetings, creating job aids, setting up processes to ensure smooth program implementation, flagging challenges and proposed solutions quickly, and refining program strategy and operations as needed
  • Develop and deliver training for Community Health Worker staff and other H+H staff on SDOH resources related to food insecurity programs
  • Collaboratively lead program design, workflow development, and implementation (clinical and non-clinical) with contractors engaged on food insecurity initiatives, monitor adherence to contract, and help position contractors for success
  • Collaborate with colleagues to develop reports, analyses, dashboards and use findings to measure program success, improve program delivery, and iterate on program goals
  • Contribute to written work products, including grant applications and decks; present program findings and successes (e.g., results of studies, analyses, evaluations); act as an ambassador of H+H’s work and facilitate bidirectional communication with external funders and other key stakeholders, as directed
  • Track literature and seminal research on food security, with an eye towards helping H+H develop and maintain sustainable resources that are progressive, meet patient needs, and promote health equity
  • Support other programmatic needs as well as administrative tasks, such as scheduling, agenda setting, and note-taking, as needed

Minimum Qualifications

A Baccalaureate Degree from an accredited college or university in Business Administration, Engineering, Social Sciences, Health Care Specialization, Physical Sciences or related programs; and,

A minimum of two years responsible experience directly related to health care program planning, research, design, operation, evaluation and analysis; or,2. A Master’s Degree from an accredited college or university in Public Health, Public or Hospital Administration, Health Care Specialization, Business Administration or related disciplines; and,

A minimum of one year experience directly related to health care program planning, research, design, operation, evaluation and analysis; or, A satisfactory equivalent combination of training, education and experience; and,

Demonstrated skills in written or oral communication.

Department Preferences

  • Knowledge in population health; concepts of community health; and social determinants of health; background and interest in food work is a plus, but not required
  • Experience in program or project management
  • Experience working in a clinical setting, such as a hospital, ambulatory care setting, or healthcare facility of any sort
  • History of serving populations in neighborhoods that bear a disproportionately high burden of illness and premature death
  • Excellent written and verbal communication skills, including the ability to present findings from data analysis concisely and effectively to a wide range of audiences and through a variety of mediums
  • Attention to detail
  • Ability to research, review, and analyze key external trends and influences from a wide range of information and distill findings into actionable population health strategies
  • Proactive, independent, and able to maintain accountability for one’s work product
  • Strong interpersonal skills with ability to build trust and maintain relationships with colleagues, community partners, and other stakeholders
