One Step Further Mental Health Rehabilitation Program located on Reisterstown Rd , Baltimore Maryland is currently looking to hire a Case coordinator Rehabilitation Counselor for their new program.

Job Description :

Help families access community resources - including support from relatives and other in the community.

• Within 30 calendar days of referral, and based on the various assessments, develop an IRP for the patient.
• Enter weekly notes for Clients for the services that have been offered to them.
• Collaborate with outside of the agencies to facilitate client care and improve PRP services.
