PathPoint is seeking a full-time Team Leader to join our Behavioral Health Division in Santa Barbara, CA. The position is currently funded through a startup grant from the Department of Health Care Services through December 2023 with the goal of the program becoming self-sustaining. The position is 40 hours per week with flexible schedule; driving is required.

In collaboration with CenCal Health, PathPoint aims to decrease historical barriers certain groups have faced to access physical and mental healthcare. As a Team Leader, you will be responsible for the project startup, quality, effectiveness, and efficiency of two new community based CenCal programs: Enhanced Care Management (ECM) and Community Supports (CS). In alignment with CalAIM, ECM Services aim to provide comprehensive field-based care management that addresses the clinical and non-clinical needs of the highest-need members enrolled in Medi-Cal managed care plans (MCPs). PathPoint's ECM Services aim to streamline care provision, reduce access disparities, and improve care quality.

As a Team Leader, you will ensure ECM/CS Services are holistic, client-centered, and trauma-informed, and the client's background, such as race, ethnicity, socioeconomic status, past housing history, gender identity, etc., are considered to create a treatment plan that best resonates with them. You will provide community-based assessments for people who may be without a home, have substance use disorder, and or a mild/moderate mental health diagnosis. You will ensure the use of client-centered tools, like motivational interviewing, to address critical mental and physical health issues (including substance use), promote ongoing health maintenance tasks (such as visiting the doctor, exercise, and medication management), and increase housing retention. In this role, you will collaborate with the Executive Director to develop policies, procedures, workflow processes, and measurable goals for the department. As a Team Leader it is your responsibility to direct, coordinate and assign duties and performance goals to team members. You will also ensure that all department activities and operations are carried out in compliance with local, state, and federal regulations and laws governing business operations.

What We're Looking For:

  • Master's degree (M.A. or M.S.) or close to graduating with Master's degree. Clinical License strongly preferred. BBS Associate Number/License (or pending)
  • Must have two years' experience working with individuals with mental illness or two years' experience in human services. Prior supervisory/leadership experience required.
  • Must have a valid California Driver's License and satisfactory DMV record.
  • Must meet all legal requirements to bill Medi-Cal
  • Must receive a criminal record clearance through the DOJ and pass pre-employment health and TB screenings.
  • Culturally sensitive and aware of differences in employees based on cultural backgrounds; aware of one's own culture and how someone's background may lead to biases; has the skills and resources needed to work with employees from all different cultures and the ability to avoid making biased assumptions based on someone's culture.

Compensation: The starting pay and pay range during the grant period is $26 per hour. If the position becomes self-sustaining after April 30, 2024, based on available funding, the pay range is expected to be $26-$29.
