Job Summary:
Support all aspects of the hospice bereavement program under the direction of the Bereavement Coordinator. Attend weekly interdisciplinary team meetings to assess risk factors and support needs associated with grief. Assist the physician and other team members in understanding the social and emotional factors related to the grieving process and end-of-life issues. The Bereavement Program Assistant supports the mission and philosophy of the Cascade Health Hospice program. Comply with applicable accreditation and regulatory requirements. This position requires proof of full COVID-19 vaccination at time of offer.
Education & License Requirements:
Minimum of a bachelor’s degree required with specialized training in grief management. Must have current driver’s license, availability of insured vehicle for transportation.
Minimum Qualifications & Experience:
Minimum one year of experience working under the supervision of a qualified, master’s-level bereavement professional or effectively delivering a range of grief and bereavement services to individuals, families, and groups. Desired skills include: ability to communicate effectively both verbally and in writing to all clients/staff/physicians and members of the community. Ability to use electronic medical records to document interactions. Appropriately respond in emergency and crisis situations. Able to work independently with minimal supervision.
Essential Job Functions:
Conduct assessments of various patient/family/caregiver situations and bereavement needs.
Respond to referrals by contacting people who staff or volunteers have identified as needing additional bereavement support.
Collaborate in the development of patient and family plans of care for bereavement. Assist hospice staff, the patient, and the caregiver/family in understanding recommendations related to the plan of care. Report findings to the Hospice Manager and the interdisciplinary team.
Prepare and document bereavement notes and progress notes as needed.
Help the Bereavement Coordinator and Volunteer Coordinator with administrative tasks, including preparation of monthly mailings, anniversary cards, and phone lists.
Contribute to the planning and preparation of annual remembrance events.
Contribute to the facilitation and organization of Bereavement Support Groups.
Provide individual grief counseling sessions as needed.
Serve as the first point of contact in the absence of the Bereavement Coordinator.
Assist the Bereavement Coordinator in providing staff, volunteer, community, facility, and bereavement training programs.
Act as a grief consultant and provide bereavement support to hospice staff. Identify available support systems and resources to reduce stress and facilitate coping with end-of-life care.
Marginal Job Functions:
Troubleshoot general office equipment problems. Clean work area. Move supplies and equipment. Perform other duties as assigned.
