Description

The Breast Center Clinical Navigator functions as an integral member of the multidisciplinary breast team of care providers who reflect a culture of cooperation, enthusiasm and mutual respect. The Navigator is responsible for coordinating the interdisciplinary team of providers to best serve the newly diagnosed breast cancer patient. This process includes assessment, documentation, education, and continuity of care for the patient and family. He/she will identify and assist patients and families in eliminating potential barriers to care by identifying and referring to appropriate resources and serve as a clinical resource with expertise in oncology care management. In collaboration with the Breast Care Team, Cancer Committee and other members of the health care team, coordinates patient care services and outreach components of the program. This includes monitoring patients through screening programs and following patients diagnosed through their cancer treatment planning and survivorship. Assists in development and provision of education, prevention, early detection programs and other activities as requested for compliance with CoC and NAPBC standards. Coordinates and schedules surgical procedures, obtaining authorization if required. There are administrative/clerical functions in this role. Collaborates in the development and implementation of performance improvement initiatives and quality indicators.


Qualifications

A college degree in nursing, PA or NP required. 2. Experience: A minimum of 3 years of experience in a hospital or community-based out-patient setting preferred. Experience as a navigator and patient educator in oncology preferred. Experience in obtaining insurance authorization for services preferred. 3. Training/Certification:.NYS RN Professional License, Oncology Navigator certification must be obtained within 2 years from hire date. Other: Demonstrates sensitivity and commitment to mission, core values and program. Must be comfortable in holding an administrative/clerical role scheduling patients for procedures/surgeries, obtaining authorizations. T
