Position Summary:

This position is responsible for providing direct care by assessing, planning, implementing and evaluating care of patients in Cardiac Rehab and supervises staff.

Responsibilities:

• Oversees the cardiac rehab program development, accreditation, and outreach.
• Performs plan of care intervention, including physical activity monitoring, medication administration, specimen collection, clinical treatments, as well as other medical care treatment.
• Documents patient care given.
• Monitors, records, and communicates patient condition as appropriate to care team, physician, patient, and family.
• Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.
• Assesses and ensures quality of services are delivered.
  

Qualifications:

• 
  Must be licensed in the State of Utah. RN, LPN, Physical Therapy Assistant, Exercise Physiologist or equivalent license or certification.
• Acute Care experience preferred.
• Previous experience with telemetry and cardiac patients preferred.
