Job Description
Care coordination is a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet the individual's health needs through communication and available resources to promote cost-effective outcomes, using evidence-based guidelines as available in daily practice. The Care Coordinator focuses on effective, safe, timely and complete transitions of care. In collaboration with the multi-disciplinary health care team, this position is responsible for the timely coordination of health care services to meet the patient's individual needs and promote quality and cost effective outcomes.
QUALIFICATIONS:
1. ASN or diploma in Nursing required.
2. Bachelor's degree in nursing, or healthcare related field preferred
3. Three (3) years of recent experience in the acute care setting
4. Minimum 1 year experience in case management/case coordination role OR 1 year of El Camino hospital clinical nursing (RN) current experience
5. Experience in managed care/continuing care preferred
6. Advanced communication skills in advocacy, negotiation, confrontation and conflict resolution
7. Computer literate, with experience in word processing, data analysis and presentation development
License/Certification/Registration Requirements:
1. Current California RN license
2. Case Management certification preferred
Salary Range:
76.49 - 99.91 USD Hourly
