Title of Position: Case Manager

Position Type: Full Time

Pay Range: Starting at $18.50 (Base $17.50 + $1.00 Locale differential) - $21.10 per hour (non-exempt/hourly) and considers preferred experience and preferred education described below.


This position also includes an additional locale differential of $1.00 per hour.

Location: Delta, CO


Description of Responsibilities: Participates as a member of the Assertive Community Treatment (ACT) team to design and implement the individual care plans for clients in accordance with policies, rules, regulations, and directives which apply. Utilizes a variety of treatment approaches and case coordination to work with a client population of persons with severe and persistent mental illness. This position will provide service coordination services for clients as assigned.


Supervisory responsibilities: No


Physical demands: Employee must occasionally lift and/or move up to 15 pounds. Employee is frequently required to walk, sit, stand or kneel and occasionally required to climb or balance and stoop. Must have ability to sit for longer periods at a computer. Employee must be able to travel between AHS locations. Possible potential exposure to communicable disease. The noise level in the work environment is usually moderate. AHS may make reasonable accommodations to enable individuals with disabilities to perform the essential functions.

Required Education, Certificates and Experience:

  • Education: Undergraduate
  • Degree Level: Bachelor’s in one of the recognized mental health professions.
  • Minimum Experience: Previous experience with substance abuse and mental health related case management and treatment. Minimum of two years human service-related experience preferred.

Preferred Education, Certificates and Experience:

  • Preferred Education: Master’s Degree
  • Preferred Certification: CAT Certification
  • Must be knowledgeable regarding patient health information and HIPAA compliance.
  • Must be organized and demonstrate the ability to follow guidelines.
  • Must demonstrate excellent interpersonal and communication skills.
  • Must demonstrate the ability to be self-directed.
  • Must demonstrate the ability to follow the organization’s confidentiality and security rules.

Required Skills:

  • Knowledge of mental illness, substance abuse, and psychopathology and associated treatment modalities.
  • A working knowledge of individual, group, family and crisis treatment approaches and their application to patients with emotional and behavioral problems.
  • Working knowledge of Colorado Revised Statutes 27-65 (Mental Health Code)
  • Working knowledge of HIPAA and 42 CFR.
  • Knowledge of community health and mental health resources and their eligibility criteria.
  • Must be a self-starter, self-motivator and possess exceptional organizational, time management and project management skills with the ability to inspire and model collaborative leadership.
  • Must be able to communicate clearly, concisely (both orally and written) with staff members, the public and other audiences.
  • Ability to organize and prioritize workload in an occasionally hectic environment with frequent interruptions.
  • Must be capable of independent judgment and action
  • Knowledge and ability to operate standard office equipment including scanner, computer, calculator, copy machine, fax, and multi-line telephone.
  • Working knowledge of Windows based computer applications including MS Word, MS Excel, MS Outlook, and ability to access the internet.
  • Ability to be helpful and sensitive to the needs of all employees, while concurrently respecting corporate policies, procedures, and priorities.
  • Interpersonal and communication skills to develop and maintain effective working relationships with all internal and external customers.
  • Ability to communicate job-related information to individuals and small groups.
  • Ability to work independently with minimal supervision; plan own work schedule effectively and multi-task and prioritize in a fast-paced setting.
  • Ability to multi-task, organize and prioritize workload in a sometimes-hectic environment with frequent interruptions.
  • Ability to concentrate in sometimes hectic and stressful settings.
  • Ability to read and understand written materials.
  • Must regard work as confidential.
  • Must have current driver’s license, qualify for commercial vehicle insurance, and maintain a driving record that allows insurability with AHS’ insurance agency.
  • Occasional travel required
  • Ability to take on additional duties when necessary.

  • CPI training within 90 days of hire
  • BLS PRO certification required within 90 days of hire
