Clinical Case Manager

Planned Lifetime Assistance Network (PLAN) of Maryland-DC, Inc was founded in 1985 by families with loved ones who were trying to manage mental illness. PLAN acts as an extension of the family to support families' efforts to provide loved ones with clinical support and other services not provided by public benefits.

The Clinical Case Manager:
We are in search of a passionate, licensed clinical social worker/counselor with the desire to improve the quality of the lives of individuals with disabilities. Our Clinical Case Manager works in the community and forms long-term therapeutic relationships with our clients to help them navigate their personal journey. This is a great opportunity to join a collaborative, knowledgeable and dedicated group of professionals.

Key Priorities and Responsibilities:
  • Work with clients and families to complete intake assessment and develop a Personal Plan of Care.
  • Provide case management services to clients based on the Personal Plan of Care. Services may include such areas as vocational, social, residential, benefit procurement, psychosocial and psychiatric needs.
  • Assess the need for and coordinate client's involvement in appropriate community resources.
  • Maintain regular contact with clients through telephone contact, e-mail, office visits and home visits. Will use own car as needed to attend client-related meetings in the community, to conduct home visits, and to transport clients when indicated.
  • Work as part of the client's team in conjunction with family and other professional providers.
  • Provide support as needed to the Trust Committee in their efforts to serve Trust clients.
What We're Looking For:
  • A master's degree in social work (MSW) or a related field.
  • Licensed as a LSCW or LSCW-C or LPC in the State of Maryland.
  • 1+ year of experience working with clients who have a serious mental illness which may include, but is not limited to schizophrenia, personality disorder, and bipolar disorder.
  • 1+ year of experience serving clients with developmental disabilities which may include, but is not limited to, autism spectrum disorder, and intellectual or learning disabilities.
