Program Overiew
NADAP's Project ACE is case management program that serves hundreds of New York City residents each year. We conduct background and clinical assessments to determine care in the community and then design and implement individualized service plans. By addressing housing, substance use, mental health and medical conditions, and legal barriers, Project ACE helps clients move toward independence and self-sufficiency.

Bilingual Russian speaker required.
Responsibilities:
The Case Manager provides case management and crisis intervention services to public assistance recipients in need of community-based treatment, employment, and/or housing services. The Case Manager also assesses the client’s needs, develops individualized plans, and coordinates services with treatment programs and community-based organizations. Weekly site/field visits to coordinate client services at treatment programs or the clients natural environment is required; position is 75% field based.
  • Coordinates case management activities and referrals to community-based resources for assigned clients
  • Completes a case management assessment to identify client needs and barriers to self-sufficiency (medical, psychiatric, alcohol/substance use treatment, housing, legal, public benefits, vocational, educational, childcare, and other needs)
  • Performs site visits to locations including the client's home, treatment program, employment site or other community-based agencies
  • Meet with clients at their home and/or treatment programs and assesses attendance, participation, and progress in treatment programs and/or work activity programs.
  • Utilizes multiple electronic database systems to monitor, report, and document services on a daily basis.
  • Collaborates with team members of other professional disciplines, including vocational counselors, social workers, treatment providers, and/or employers to ensure that client is meeting goals and objectives of the service plan.
  • Other tasks and duties related to case management services.
Qualifications:
  • Education: Bachelor’s degree in social work, psychology, sociology, criminology, criminal justice, rehabilitation counseling, or other related field is required.
  • Bilingual Russian speaker required.
  • 1+ years of social service experience, which can include internships, research, field study, and/or volunteer work preferably including case management experience.
  • Knowledge of substance use , mental health, and/or medical treatment modalities and New York City’s treatment system
  • Excellent verbal and written communication, teamwork, and organizational skills.
  • Proficient in MS Office; Computer savvy- experience using computers and smartphones, navigating the internet, and word processing
