Qualifications:

A Bachelor’s Degree in Social Work or a related field is required (Master’s Degree preferred), as well as at least two years’ relevant experience with an adolescent population in a mental health, residential treatment, community group home setting is required. A valid driver’s license, reliable car, and a good driving record are also required.
Responsibilities:
The primary responsibilities of the Case Manager position include: assuring resident safety, providing a full range of case management services to residents and families, implementation of treatment planning, ensuring that residents receive trauma informed services, partnership with the multi-disciplinary team, and monitoring of assigned youths' placement from admission to discharge. Position requires the liaison with collateral agencies and maintaining the client file assuring the record meets regulatory guidelines.
