Job Description:

Salaried, fulltime (35 – 40 hours per week) Case Manager position for a small nonprofit housing program with budget under $1 million and staff of 4 individuals.

Case Manager provides intensive case management to formerly homeless families. This position reports to the Program Manager. Position may require some evening work.

Responsibilities include but are not limited to:

• Case Management to 15 families; 7 in Permanent Supportive Housing, 8 in the Housing Initiative Program (HIP)
• Develop individualized goal plans
• Create and monitor budgets
• Provide referrals to needed resources and/or community partners
• Maintain current case notes and files on each client
• Monitor client accountability
• Maintain weekly contact with each client (home visits, office meetings, etc.).
• Attend monthly meetings with HIP Clinical Supervisor
• Participate in staff meetings and other community meetings as needed
• Serve as one of the point of contact for family landlords (HIP and HOC)
• Attend and help plan Life Skills meetings
• Maintain accurate and timely HMIS data and run reports as needed
• Other duties as assigned

Preferred Qualifications:

• Bachelor’s Degree in Social Work or related field work required. MSW preferred.
• A minimum of three years of relevant experience, especially with low-income and homeless population.
• Familiarity with local social services support programs.
• Must be able to work independently, possess strong written and communication skills, and be comfortable with computer work.
• Must be able to lift 25 lbs.
• Must be available to work evenings as needed
