Title: Case Manager for Women in Residential Treatment Facility- ARC Fond du Lac

Our Mission: To provide community-based, integrated, multidisciplinary services within the family context. ARC works as an advocate to promote the system-wide adoption of women-responsive approaches to improve successful outcomes for women and their families. ARC services are family-centered, strengths-based and evidence based best practices. ARC recognizes the trauma experienced by the women referred to ARC programs, and works to create safe environments for treatment, to empower women to exercise choice in their treatment, to involve peers, family, and the community, and avoid retriggering traumatic experiences.

Our Team: ARC Fond du Lac is an innovative 16-bed residential treatment facility for women located in Fond du Lac. ARC Fond du Lac provides substance use disorder treatment and community corrections programming for women. The program uses comprehensive case management and treatment services that are gender and culturally competent to meet the needs of women and their families affected by substance use disorders. Responsibilities of the Case Manager:

· Provide case management services for assigned clients

· Create comprehensive treatment and continuing care plans with clients and update weekly

· Attend weekly clinical staff meetings with clinical supervisor and monthly staff meetings with the medical director

· Complete monthly and discharge reports

· Create relapse prevention plans and coordinate continuing care services for all clients

· Meet with clinical supervisor weekly or as license mandates

· Facilitate/Co-facilitate assigned groups

· Maintain documentation according to DHS 75.54 regulations

Qualifications:

Master’s degree in social work, Counseling, or related field preferred

· Bachelor’s degree required

Salary: From $42,000 based on experience, education and level of licensure
