PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Interim Housing team as the Case Manager at the Highland Gardens Location.

ABOUT OUR TEAM
PATH’s Interim Housing programs provide low-barrier, high-tolerance settings for program participants, referred to as guests, who are experiencing homelessness and have a variety of complex health, functional and social needs. These home-like environments are coupled with support and case management services to assist guests with developing a plan to break the cycle of homelessness, stabilize in housing and reintegrate into the community.

WHAT IS A CASE MANAGER
As part of the Interim Housing team, the Case Manager assists guests with building skills, reducing mental health and functional barriers to
obtaining and maintaining housing, and strengthening community social service linkages necessary for the guest to
fully integrate into the community once they leave interim housing.

Position Responsibilities include:

  • Provide field based/mobile case management services in the areas of, but not limited to: Independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment, linkage to community providers for substance abuse, primary and mental health care, and all other services needed to assist clients in reaching their goals.
  • Provide strengths-based case management and service coordination services designed to assist clients in obtaining and maintaining stable employment and housing.
  • Develop individualized treatment plan in collaboration with client addressing short-term and long-term goals.
  • Provide on-going case management support to assess progress and ensure treatment plan outcomes are met or changed as needed.
  • Provide services focused on enhancing the clients’ ability to independently problem solve, utilize effective coping skills, and manage and self-coordinate own care.
  • Utilize evidence-based practices in service delivery such as intensive case management, Motivational Interviewing, Harm Reduction, Critical Time Intervention and Housing First practices
  • Perform housing location and retention services by conducting neighborhood canvassing, landlord outreach, unit inspections and providing support to resolve tenant & landlord disputes
  • Work closely with program employment specialists to assist clients in obtaining and maintaining employment
  • DocumentationMaintain documentation standards as set forth by the program contract and PATH policies, including maintaining compliance with HIPAA policies and practices

Complete progress notes on every face to face/ telephone contact with client or collateral contact by the next business day at 5pmComplete intake, assessment, service plans in coordination with client and within the documentation standards set forth by the program contract and PATH
WHAT YOU BRING

We’re looking for candidates that:

  • Ability to maintain and execute confidential information according to HIPAA standards.
  • Demonstrated knowledge and/or experience with case management techniques, including critical time intervention, harm reduction strategies, crisis intervention techniques and motivational interviewing.
  • A high level of tolerance and understanding for individuals who present for services with urgent multiple case management and health needs.
  • Ability to work independently and within a team environment and exercise mature judgment.
  • Ability to provide community and in-home based services on a regular basis.
  • Strong written and verbal communication skills.
  • High level of motivation and self-starting demeanor with the ability to coordinate multiple projects/ tasks simultaneously in a high-pressure environment.
  • A high level of tolerance and understanding for individuals who present for services with urgent multiple case management and health needs

PREFERRED QUALIFICATIONS

  • Two (2) years of experience working in homeless services, mental health, substance use or co-occurring disorders, or with vulnerable populations providing intensive case management services
  • Permanent supportive housing experience.
  • HMIS experience.

MINIMUM QUALIFICATIONS

  • All levels of education and experience welcome.

MANDATORY REQUIREMENTS
Regardless of qualifications, candidates must:

  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Provide proof of full COVID-19 vaccination
  • Successfully complete the following as a condition of hire: Tuberculosis Test

  • Have reliable transportation
  • A valid driver’s license

COVID-19 VACCINATION
Effective 11/01/2021 PATH will require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of full vaccination as part of the pre-employment credentialing process.
