Case managers are responsible for assessing, planning, implementing, monitoring, and evaluating options and services required to meet an individuals health needs. Case Managers are responsible for facilitating the timely and appropriate transition of patients along the healthcare continuum. The focus of their efforts is to access quality care for patients by collaborating with payers and providers in developing, implementing and evaluating quality, cost -effective treatment options delivered in the most appropriate setting. Educating members and providers to options in the managed care arena is also part of the case managers daily duties.
The Case Managers endeavor to avoid duplication and fragmentation of care that can frustrate all parties involved-patients, payers, and providers. Identifying and helping bridge the gaps and barriers in the Health System and community resources is also part of the Case Managers focus. Health System Disease Management initiatives are fully integrated into the Case Management position. Specific Case Management interventions are performed for the purpose of meeting Medical Management objectives.

Qualifications:

- Education: Registered Nursing degree or diploma.
- Licensure: Unrestricted license as a Registered Professional Nurse or Social Worker in the State of Missouri.
- Experience: Minimum of five years diversified clinical experience in nursing or other related field i.e.: Medical Social Services.
- Certification: Case Management or related certification preferred or attained within 24 months of hire.
- Other skills & knowledge (skills, knowledge, abilities):
- Excellent interpersonal skills.
- Above average human relations, verbal and written skills in order to communicate effectively and obtain cooperation from all corporate levels, providers, patients, and families.
- Ability to work independently.
- Advanced problem-solving, teambuilding and negotiation skills. Keyboarding and data entry skills.
- Knowledge of healthcare delivery systems, basic utilization management, and quality improvement concepts and principles.
- Extensive background and knowledge of current medical/surgical patterns of practice.
- Broad-based knowledge and clinical expertise in assessing, analyzing, intervening, and evaluating patient care.
- Ability to work in stressful environment: ability to prioritize and manage multiple tasks, function within an ever changing environment, coordinate services for multiple patients with a variety of ages and diagnoses.
- Working Conditions, Mental and Physical Requirements: Highstress, fast-moving environment requiring flexibility and adaptability in multiple environments.
- Ability to balance multiple tasks at once and prioritize effectively and efficiently.
- Ability to concentrate in areas of high activity.
- Work settings include hospital areas and cubicles.
- Must be able to move laptop to multiple areas and access LAN connections.
- The job is generally Monday through Friday day office hours with rotational call for all hours when office is closed (evenings, nights, weekends and holidays).
- Equipment Used: Routine office equipmentLaptop or desk top computer, copy machines, fax machines, digital pagers, telephones, and cell phones.
