Position Summary

Position provides case management, care coordination and resource navigation for patients and families within the Autism Clinical Center.

Job Description

Primary Duties & Responsibilities

Effectively coordinate client care with other service providers and community resources to assure continuity of care, and navigate provider waitlists (including the Regional Center to establish services).

Maintain active listing, and assist with referrals to (including, but not limited to):

• In-home and clinic-based ABA Therapy
• Parent training
• Psychotherapists
• Parent support groups
• Sibling support groups
• Summer camps
• Social skills groups
• PCIT
• Respite care
• Residential facilities
• CMHC wrap around resources, and how to navigate the process.

Provide psychoeducation on Autism and other developmental disorders after a patient receives a diagnosis of Autism. Offer emotional support to caregivers after evaluations.

Display a professional demeanor and excellent communication skills, written and verbal, with the ability to relate to people of varying socioeconomic class with differing lifestyles or cultural values, and to interact with community based service organizations, police, courts and schools, respectfully and professionally.

Documentation

• Complete clinical documentation and psychosocial reports, including the family’s social needs that case management can help address (especially any information that psychiatric evaluations may not include).
• Review patient electronic medical record to assist and inform patient treatment.

Work within Multidisciplinary Treatment Team

• Attend Division of Child Psychiatry trainings as appropriate.
• Remain current in research and knowledge of competencies related to the case management duties assigned.
• Provide multidisciplinary collaboration between all members of the treatment team to facilitate and coordinate quality patient care in a timely, cost effective manner.
• Function well in a team environment while contributing positive support to colleagues and peers.
• Demonstrate willingness to seek/accept direct supervision as well as respect productive feedback from colleagues and others.

Preferred Qualifications

• Master’s level social worker with a minimum of two years of experience working with a youth-serving program.
• Familiarity with community resources in St. Louis City and St. Louis County.
• Ability to operate from a humanistic and holistic perspective on working with youth and their families with emotional distress, trauma histories, and mental health conditions.
• Ability to work with clients from diverse racial and socioeconomic backgrounds.
• Excellent time management and organizational skills and an ability to work independently and self-directed as well as with the team.
• Flexibility of schedule in order to meet the needs of clients and community partners.
• Ability to work in stressful situations, good crisis intervention skills, reliable, working knowledge of DSM-V

Required Qualifications

Master’s degree in social work, counseling, psychology, or a related field. An equivalent combination of experience and education may substitute for a master’s degree.

Salary Range

$50,600.00 - $75,800.00 / Annually

The salary range reflects base salaries paid for positions in a given job grade across the University. Individual rates within the range will be determined by factors including one's qualifications and performance, equity with others in the department, market rates for positions within the same grade and department budget.
