JOB DESCRIPTION FOR PERSONAL CARE WORKER
Position Summary: An individual who completed CBRF training and is competent to perform assigned functions of supportive care to the residents inside of our Adult Family Home.
QUALIFICATIONS:
- Must be at least 18 years of age.
- Must have completed CBRF training program and competency.
- Ability to carry out directions, read and write.
- Have sympathetic attitude towards the care of the sick and elderly.
- Maturity and ability to deal effectively with demands of the job.
RETURN DEMONSTRATION OF SKILLS
Training shall be provided for each skill the CBRF caregiver is assigned and shall include a successful demonstration of each skill by the employee to the qualified trainer under the supervision of the supervisor prior to providing the service to the residents.

JOB DESCRIPTION
- CBRF caregiver provide a service to individuals in our Adult Family Home, when needing assistance caring for themselves as a result of old age, sickness, disability and/or other functions. Supportive care may include assistance with activities of daily living, housekeeping, laundry, meal preparation, transportation and companionship.
- CBRF employees are responsible for ensuring that service is delivered in a caring, professional and respectful manner in accordance with relevant agency policies and standards.
REPORTING RELATIONSHIP
- Reports to supervisor and house lead
DUTIES: CBRF Caregiver shall perform/do all the following duties:
- Perform tasks assigned by Supervisor
- Report in writing to Supervisor on each shift
- Promptly report any significant changes observed or reported in the client’s condition to the Supervisor and
- Confer as required with Supervisor regarding residents progress
Covered personal care services include:
- Assistance with bathing
- Assistance with getting in and out of bed
- Teeth, mouth, denture and health care
- Assistance with mobility and ambulation including use of walker, care or crutches
- Changing the recipient’s bed and laundering the bed linens and the recipient’s personal clothing
- Skin care excluding wound care
- Care of eyeglasses and hearing aids
- Assistance with dressing and undressing
- Toileting including use of use and care of bedpan, urinal, commode or toilet
- Light cleaning in essential areas of the home used during personal care services
- Meal preparation, food purchasing and meal serving
- Simple transfers including bed to chair or wheelchair and reverse
- Accompanying the recipient to obtain medical diagnosis and treatment
WORKING ENVIRONMENT
Works both indoors in the agency office and in the field with clients and referral sources.
REQUIRED KNOWLEDGE:
- Knowledge of personal care and home management skills.
- Knowledge of principles and processes of providing client and personal care services, including needs determinants, meeting quality standards and evaluation of clients’ satisfaction.
- Knowledge of English language.
- Knowledge of the information and techniques needed to diagnose and treat injuries including emergency first aid and CPR.
- Knowledge of clerical procedures such as maintaining records and completing forms.

- Ability to stand, walk, lift, bend, kneel, stoop, reach, use hands and fingers, crouch, talk, hear and see.
- Ability to drive a motor vehicle.
- Mental fortitude and stability to handle stress.
- Ability to work for an extended period while standing and being involved in physical activity.
- Heavy lifting.
- Ability to participate in physical activity.
The personal care worker shall be assigned by the supervising registered nurse to specific recipients for which the personal care worker has been trained. The personal care worker’s training for these specific tasks shall be assured by the supervising registered nurse. The personal care worker is limited to performing only those tasks and services as assigned for each recipient and for which he or she has been specifically trained.

I further understand that my status a s an “at will” employee may not be changed except in writing and signed the President of Visions of Helping Hands Personal Care Agency.
I understand that the information I come in contact with during my employment is proprietary to Visions of Helping hands Personal Care Agency and accordingly, I agree to keep it confidential, which means I will not use it other than in my performance of my duties or disclose it to any person or entity outside Visions of Helping Hands Persona Care Agency. I understand that I must comply with all the provisions of the handbook to have access to and use Vision of Helping Hands resources. I also understand that if I do not comply with all provisions in the handbook, my access to those resources may be revoked, and I may be subject t disciplinary actions up to and including discharge.
I further understand that I am obligated to familiarize myself with Visions of Helping Hands Personal Care Agency’s safety, health and emergency procedures as outlined in this handbook or in other documents.
