Woodhaven Nursing Home is a 143 bed Skilled Nursing Facility located in Port Jefferson Station, NY. We are seeking Full Time and Part Time CNA for all shifts: Day shift is from 7am to 3pm, Evening shift is from 3pm to 11pm and Night shift is from 11p to 7am. Responsibilities:

• Assists in the preparation for admission of residents such as ensuring the bed is made, personal hygiene products delivered to room such as wash basin, toothpaste, toothbrush, lotion, etc.
• Assists with transfers, weight, introduction, unpacking of personal items for resident.
• Assists in and accompanies residents in transfer and discharge procedures.
• Provides personal care which may include sponge bath, bed bath, shower. oral hygiene, combing hair, skin care, shaving, dressing and undressing.
• Provide toileting and incontinence are to residents per care plan and when requested or needed and does so in a timely manner.

Requirements/Qualifications:

• High School Diploma or equivalent
• Must have NYS certification.
• Must be able to follow directions, both oral and written and work cooperatively with other staff members
• Possess a genuine interest and concern for geriatric and disabled persons.
• Treats residents, families and co-workers with respect and dignity
• Must be vaccinated against Covid-19. Booster a plus
