Job Summary
Heritage Hospice Home Health Aid is committed to the philosophy, goals, and objectives of the hospice concept. As a member of the interdisciplinary team, the Home Health Aide (CNA) will provide personal care services to the terminally ill patient and perform related task in the home setting in accordance with the plan of care as assigned by the primary nurse

Qualifications
1. High school diploma or GED
2. Previous experience as a Certified Nursing Assistant and/or completion of a certified training program.
3. Current Registration as a Certified Nursing Assistant in Georgia
4. Emotional maturity and ability to lift, push or pull an min. of 50lbs.
5. Dependable transportation
6. Working telephone where can be reached
7. Obtain a score of at least 80% on Heritage Hospice CNA examination at the time of hire.
8. Be certified by RN annually according to the CNA skilled evaluation.

Responsibilities
1. Responsible to RN Case Manager for caring out the duties assigned in order to increase patient's level of comfort, and to maintain personal hygiene and a safe environment for the patient.
2. Under the direction of the RN Case Manager , The CNA will help achieve good quality of life.
3. Recognize and report any changes in the mental or physical status of the patient promptly to the RN Case Manager or Home Hospice Coordinator if the RN Case Manager is unavaliable.
4. Provide respite relief for primary caregiver as requwsted by Home Care Coordinator.
5.Partipate in Interdiciplinary Team meeting as assigned.
6. Assist the Registered Nurse with specific assigned procedures.
7. Complete appropriate records in a timely manner.
8. Other Duties as assigned
