This role is a contractor position or w2 employee, inclusive of benefits.
- Screens patients to determine need for intervention and initiates referrals as appropriate
- Evaluates patients and interprets data
- Develops treatment goals and plans based on evaluation data, rehabilitation potential and discharge plans
- Involve patient, family and other care team members in the creation and development of treatment goals
- Implements patient treatment programs which are safe, effective, and efficient
- Updates/ modifies treatment plans according to patient needs and progress
- Terminates treatment appropriately (data based), and develops appropriate discharge and post discharge plans
- Completes evaluations, progress notes, reassessments, discharge summaries, and third party reimbursement reports per established departmental, institutional and regulatory requirements within required time frames
- Attends team and family conferences; interprets pertinent findings and contributes to the development of a multidisciplinary plan of care
- Pursues professional growth and development through seminars, workshops, and professional affiliations to maintain certification/licensure, and/or keep abreast of latest trends in the field
- Demonstrates competence with daily operations within discipline such as managing schedule, successfully prioritizing patient care
- Assists in the development, implementation and assessment of new programs, clinical research and special community projects needs/opportunities arise
- Participates in performance improvement projects; makes recommendations for improvement
- Communicates effectively with all levels of staff.
- Fosters the development of teamwork within the department while meeting the needs of departments being served
- Examines, evaluates, plans and provides quality therapy treatments as prescribed that meet patient individual needs using recognized standards of professional practices
- Collaborates with the healthcare team and involves patient and/or family in the development and implementation of individualized therapy programs designed to restore, reinforce and enhance performance
- Identifies patient’s specific needs, making referrals to other disciplines to address the comprehensive needs of the patients
- Instructs, educations and trains patients and caregivers in the skills and functions necessary for promoting patient independence and productivity
- Contributes to the monitoring of the quality and appropriateness of patient care delivered
- Completes necessary documentation and maintain clinical record for each client during the patient’s length of stay
- Maintains superior interpersonal communication skills
- Ensures all documentation of care provided written professionally
- Exhibits both knowledge and understanding of therapy practice
