Job duties:
- The Family Engagement Child Team Meeting Facilitator (CFTM) will provide an array of CFTM services as assigned. This role will be key in the development of the Agency's CFTM practice region wide.
Identify/Assess the need for a CFTM and hold a Pre-CFTM meeting with the family to begin preparation for the CFTM and submit referral in Child and Adults Protective Services System (CAPSS).- Provide provide an array of Family Engagement services through preparation, planning and facilitating CFTM's .
Engages families in the family finding process to include Genograms, Ecomaps, Connectedness Charts and CLEAR or Seneca Searches.
Documents CFTM's accurately in CAPSS, completes IV E and other required documentation.
Increase community and provider network awareness of DSS family engagement practices such as child and family teaming.
Will be required to perform Red Cross shelter/emergency response duties as directed.
Agency Requirements:
A Bachelor's Degree in Social Work, Social Welfare or Behavioral Science and one (1) year of professional experience in Human Services or Social Service programs.
A Bachelor's Degree in any other field and two (2) years of professional experience in Human Services or Social Service programs.
A Master's Degree in Social Work, Social Welfare or Behavioral Science. Selected positions may prefer supervisory experience.
A Master's Degree in any other field and one (1) year of professional experience in Human Services or Social Sciences.
