General Summary
    The Chaplain, as a member of the interdisciplinary team, serves as the spiritual counselor and is responsible for the provision of spiritual support to hospice staff, patients and families in keeping with their desires and beliefs. The Chaplain also serves as the Hospice Bereavement Coordinator. The Bereavement Coordinator provides bereavement services to Hospice patient caregivers and families; assesses and provides bereavement counseling for those at risk for pathological grief.
Essential Duties and Responsibilities
  • Assesses patient and family spiritual, psychosocial, and environmental needs and develops an individualized spiritual plan of care for each assigned patient and communicates the plan to the hospice interdisciplinary team.
  • Implements the plan of care using objective, measurable goals with time limited outcomes
  • Assesses bereavement risk on admission per hospice policy.
  • Assists family with funeral arrangements by conduction arrangements for funereal or memorial services when indicated.
  • Communicates with patient’s clergy by contacting them as appropriate and as desired by patient and family.
  • Identifies community resources for delegation of specific spiritual care needs of the patient and family.
  • Maintains productivity standards by performing duties in an efficient and effective manner.
  • Participates in weekly hospice interdisciplinary team meetings.
  • Works with staff, clergy and community groups to enhance sensitivity to the spiritual concerns of patients and families experiencing terminal illness.
  • Attends in-services as related to spiritual care.
  • Completes documentation for chaplain visits timely. Documentation is appropriate and describes the interventions and progress toward goals.
  • Bereavement Coordinator:

  • Develop plan of care for bereavement services that:
    • Reflects family needs
    • Performs the bereavement risk assessment after the death of the patient as per hospice bereavement policy.
    • Develops and individualizes the plan of care following hospice bereavement policy, including the frequency and type of services planned.
  • Implements the plan of care using objective, measurable goals with time limited outcomes
  • Completes documentation in the bereavement chart timely. Documentation is appropriate and describes the interventions and progress toward goals.
  • Revises bereavement care plan as indicated by ongoing assessment of the bereaved.
  • Provides direct bereavement counseling that allows for effective interventions in the time of grief related crisis to the bereaved, encouraging effective coping with grief.
  • Provides information, referrals, support, and documentation to patient caregivers, significant others and families and other members of the health care team regarding the mental/emotional aspects of their adaptation to pre or post patient death related bereavement needs.
Job Qualifications
    Education
      Necessary requirements to be ordained, commissioned or credentialed according to the practices of an organized religious group, and has completed, one unit of Clinical Pastoral Education (CPE); OR has a minimum of a bachelor’s degree with emphasis in counseling or related subjects and has, within ninety days of hire, completed specific training to include: common spiritual issues in death and dying; belief systems of comparative religions related to death and dying; spiritual assessment skills; individualizing care to patient beliefs and varied spiritual practices/rituals.

    Work Experience
      Experience in working with patients and families dealing with life-threatening illness and death. Hospice experience preferred.

    Certification/License
      Pastoral certificate or diploma required. Current driver’s license and declaration of automobile liability insurance required.
      BLS through the American Heart Association required.
Mental/Physical Requirements
    Ability to stand and walk for long periods of time and to walk up and down flights of stairs. Ability to receive and express detailed information through oral communications, visual acuity, and the ability to read and understand written directions. Lifting up to 35 lbs. Excellent interpersonal and organizational skills. Ability to safely operate an automobile. Drive: 2-4 hrs. per day Must be able to maneuver around architectural barriers and obstructions in patient's home. Must be able to tolerate temperature changes, smoke, fumes, dust and pets found in patient's home.
Working Conditions
    Frequent exposure to infectious disease and hostile persons at times. Frequent exposure to communicable disease via blood and other body fluids. Minimal physical discomfort. Various community environments.
