The Quality and Patient Safety Advisor role will support the following:
- The development and oversight of quality improvement initiatives with the relevant lead managers in Community Healthcare East which will result in the delivery of better and safer care
- Systems and processes to support and coordinate clinical audit activity within clinical services and departments
- Work with the wider QPS team to develop training programmes required to assist compliance and continuous quality improvement, training for incident and risk management in line with HSE policy and guidance across Community Healthcare East
- Work with managers to design quality improvement programmes / initiatives, data processing and analysis which will support implementation and monitoring of quality and patient safety
- Analyse and summarise performance improvement data and report on findings and outcomes
- Participate in the development of local policies and procedures required to assist compliance with standards
- Support the governance structures and processes for quality and safety in place to enable risk assessment, reporting and management in Community Healthcare East services
- Support systems and processes for incident management including communication and escalation of serious incidents (internally to Community Healthcare East management and to external agencies e.g. HIQA, Mental Health Commission, Clinical indemnity scheme)
- Facilitate and support Community Healthcare East Quality and Patient Safety committees (to include Incident Management Learning and Support)
- Facilitate and support the system in place for reporting/inputting of all incidents to the National Incident Management System (NIMS) – hosted by the Clinical Indemnity System
- Support the frontline mangers in the maintenance and updating of Risk Registers within Community Healthcare East
- Provide advice and support on all aspects of risk and incident management within Community Healthcare East
- Participate in incident review teams and where required, lead and support system analysis reviews in line with the HSE Incident Management Framework 2020
- Facilitate and support processes for tracking implementation of recommendations from major reports, reviews, investigations and clinical audits
- Contribute to maintaining a repository of organizational learning from safety incident and risk management and disseminate learning to enable mitigation of future risk
- Support service managers in ensuring that, where learning is identified, the necessary changes are put in place and all corrective actions are closed out so to prevent recurrence.
- Contribute to the development of performance indicators for risk management system which can be monitored (quality and safety profile)
- Provide reports as required to the Quality and Safety Manager and service managers on performance in relation to national and local quality and patient safety KPI’s
- Participate in Community Healthcare East cross divisional collaborative working on QPS related initiatives / committees to support service improvement
- To promote and participate in the implementation of change
- To represent the Quality and Safety Manager on committees and groups as required
- To develop and maintain positive working relationships with key stakeholders both internal and external
- Undertake special assignments and reviews as directed
- To carry out any other appropriate duties or assignments as requested by the Quality and Safety Manager including deputising as appropriate
- Participate as appropriate in any national / regional group established to assist in improving quality and patient safety.
- Have a working knowledge of the Health Information and Quality Authority (HIQA) Standards, the Mental Health Commission Quality Framework, Regulations, Rules and Codes as they apply to the role for example, Standards for Healthcare, National Standards for the Prevention and Control of Healthcare Associated Infections, Hygiene Standards etc and comply with associated HSE protocols for implementing and maintaining these standards as appropriate to the role
- Have a working knowledge of the HSE’s Incident Management Framework 2020, Integrated Risk Management Policy 2017, Open Disclosure policy 2019.
- To support, promote and actively participate in sustainable energy, water and waste initiatives to create a more sustainable, low carbon and efficient health service
