The GP Lead role is central to the future of community based and integrated care and is a unique opportunity to help mould how GPs and other care professionals interact and collaborate in the management and delivery of services at network level going forward. Optimising integrated service planning will support better coordinated care, allowing gaps in services to be identified and services to be put in place.
Roles & Responsibilities
Clinical Leadership
The CHN GP Lead will:
- Act as a member of the CNH Management Team and provide clinical expertise and input as well as representation for all GPs within the CHN.
- Attend monthly CHN Management Team meetings.
- Contribute to the identification of skills gaps and training requirements for GPs within the CHN.
Planning, Performance and Quality Assurance
The CHN GP Lead will:
- Lead in conjunction with the Network Manager and Assistant Director of Public Health Nursing biannual Network Service Planning workshops attended by GP’s, members of the CHN Team and other community service staff as appropriate. Represent trends within General Practice in the CHN and contribute to the planning and prioritisation of CHN services and funding in line with population needs data.
- Provide input to the development, and promote the achievement, of key performance indicators (KPIs) relating to General Practice within the CHN.
- Support the completion and interpretation of CHN population needs assessments and subsequent dissemination of information and related findings to GPs within the CHN.
Service Development and Improvement
Through participation in monthly CHN Management Team meetings and bi-annual Network Service Planning workshops etc., the CHN GP Lead will:
- Work collaboratively with the Network Manager to promote multi-disciplinary working, coordination of service delivery and development of appropriate care pathways within the CHN.
- Contribute to the development and delivery of CHN Operational Plans;
- Contribute to the development of Primary Care services and the integration of Primary Care services with all other services and care divisions (including, but not limited to, acute hospitals, mental health, social care, and health & wellbeing) to optimise outcomes for service users.
- Provide clinical guidance, advice and input to the Network Manager and the CHN Management Team in relation to quality and service improvement initiatives, and the implementation of National Clinical Programmes, national standards and local protocols within the CHN.
- Collaborate with the CHN Management Team to develop professional relationships, innovative solutions and multi-disciplinary approaches to challenges within the CHN.
- Work with the CHN Management Team to develop procedures for the management of complex cases.
- Work with the Executive GP Lead and Network Manager to identify and address issues within General Practice in the CHN, inclusive of those experienced by single-handed GPs in practice.
Communications:
The CHN GP Lead will:
- Establish a process of two-way communication with GPs in the CHN.
- Collate and present the views and experiences of General Practice within the CHN at CHN Management Team meetings.
- Promote the development and maintenance of relationships between GPs and Primary Care teams within the CHN in order to deliver high-quality, coordinated care.
- Ensure relevant updates and documentation from CHN Management Team Meetings are shared with GPs within the CHN.
- Represent the views and experiences of General Practice within the CHN to the Executive GP Lead and keep the Executive GP Lead appraised of issues at PCT and CHN level;
- Liaise with other CHN GP Leads within the wider Community Healthcare Organisation or as appropriate;
- Maintain direct contact with HSE management and act as a conduit for communication between GPs in the CHN and HSE management.
- Be a spokesperson for the relevant Community Health Network in relation to Community Services (e.g. with the media, professional groups and other relevant fora), where appropriate.
