Essential Job Functions:
- Records services and outcomes, submits confidential reports as required.
- Identifies proper forms for Aftercare Advocacy documentation and correct placement in client record.
- Documentation of activities including outcomes, and plans, using DAP format.
- Produce appropriate reports to judges, probation, child-family agencies and referral sources in a timely efficient manner.
- Adheres to HIPAA, safety and security rules and procedures.
- Maintains professional communication and attitude with community providers, program staff, residents, their children and family members.
- Coordinate with staff, community providers, and clients, their children and family positive interpersonal skills to obtain resources and resolutions to client related needs and services.
- Coordination of Case Management Services and Needs.
- Work with WARM @ VCS interdisciplinary treatment team, community providers and partnerships to ensure best practices with client case planning and services.
- Maintain on-going contact with courts, probation, Department of Children, Community Partnership for Children and other client linkages to maintain client outcomes. Contact made by telephone, in meetings, letters, and other paperwork important to client services.
- Develop treatment planning, on-going assessment, and discharge planning.
- Assists in coordination of performance plans, monitor client activities, provide and refer clients to develop sober life skills in the areas of daily functioning, employment, education, and self-care needs.
- Assists client and their children with transitional services for aftercare. Introduce and work with clients for ongoing recovery support through Strengthening Families Program, a nationally recognized parenting and family-strengthening program for high-risk families.
- Assists clients with the Community Café and other community resources to support their recovery.
- Works with clients as they prepare to transition back into the community from WARM @ VCS, stressing the use and importance of evidence based curriculum of Motivational Enhancement Therapy, Cognitive Behavioral Therapy, Seeking Safety Curriculum, Moral Reconation Therapy and Living in Balance Curriculum.
- Assists clients after transition back into the community with housing, employment, education.
- Makes visits to the home of the client; assists with crisis stabilization referrals if needed.
- Assists and encourages AA/NA community support.
- Maintains records and statistics for quality assurance and other purposes.
- Performs related work as required.

Qualification Requirements: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Minimum Education and/or Experience: Possession of a high school diploma or GED. One year of related human services experience including outreach and referral services. Case management experience a plus. Thorough knowledge of the recovery support community a plus. Graduation from an accredited school with a Bachelor's Degree in a related field preferred; currently certified as an Addictions Professional in the State of Florida preferred. Current valid driver's license in the state of Florida is required.
Knowledge/Skills/and Abilities:
- Must be knowledgeable and demonstrate competence in current principals and including connecting clients with community resources, discharge/transition planning, and familiarity of aftercare programs in Florida, which are clinically specific to substance abuse.
- Must be familiar and demonstrate competency in effective case management techniques and behavioral and clinical management of clients.
- Have knowledge of typical substance abuse diagnoses.
- Demonstrate familiarity with software programs used at SMA, or the ability to learn them in a reasonable period of time.
- Demonstrates safe operation of computers and associated hardware.
- Knowledgeable of HIPAA and State confidential regulation as they pertain to health information implements policies in daily work assignments with staff, community providers, clients and their family.
- Ability to communicate effectively with suicidal, mentally, emotionally, and/or psychologically disturbed clients in person and over the telephone.
- Ability to defuse potentially dangerous situations, and remain calm in an emergency.
Necessary Special Requirements: Possession of a valid Florida driver's license, acceptable driving record, and proof of personal automobile insurance if required to drive an SMA vehicle and/or use a personal vehicle for SMA business. Complete State of Florida mandatory background screening prior to start of employment. Complete SMA required training during the first six (6) months of employment and updated if required.
Physical: Mobility and ability to bend and reach during an 8-12 hour day. Able to lift minimum 10 pounds. Visual and auditory acuity sufficient to evaluate, intervene, treat, and record client health care needs. Fine motor skills for legible and accurate charting, daily correspondence and presentation, either manually or orally. Work endurance ability to work 8-12 hour shifts with a meal break, as possible. Routine 8-12 hour shifts. Hours and days off may vary. Extra hours may be required. Work assignment locations may vary.
