Job description
We are looking for a Clinical Dietitian to join our team. The ideal candidate will have a background in dietetics and home health, and will be responsible for providing nutritional counseling and education to patients in their homes. The Clinical Dietitian will also be responsible for creating individualized nutrition plans, monitoring patient progress, and providing support to family members. Responsibilities:
- Develop and implement nutrition care plans for patients.
- Educate patients and their families on nutrition and dietary needs.
- Monitor patient's progress and adjust nutrition care plans as needed.
- Provide nutritional counseling to patients and their families.
- Assess nutritional needs of patients in home health settings.
- Provide nutrition education to the community.
- Collaborate with other healthcare professionals to ensure optimal patient outcomes.
Requirements:
- Equivalent experience will be accepted in lieu of the required degree or diploma.
- Bachelor's: Food and Nutrition, Dietetics or related field. or equivalent education/experience
