Job description
The Clinical Pharmacist prepares medications by reviewing and interpreting physician orders; controls medications by
monitoring drug therapies and advising interventions. This position is also responsible for providing education and
professional development for pharmacy staff regarding pharmaceutical care and clinical guidelines.
- Accurately interprets, profiles and fills physicians’ orders. Ensures all medications filled, checked and delivered in a
timely manner. - Monitors drug therapy for duplicates, contraindications, interactions, incompatibilities, allergies, and dosing errors.
Documents interventions as needed. - Review and monitors drug therapy that requires dosage adjustment per physician order. Intervenes with physician to
clarify medication orders, dosage, frequency, route as needed. Documents changes in Medical Record as per telephone
order. - Aseptically prepares intravenous fluids, irrigation solutions, additives, chemotherapy and other sterile products.
Maintains appropriate documentation. Dispenses control drugs and maintains accurate records and inventory as
required by law and pharmacy policies. - Maintains inventory of pharmaceuticals and supplies, and ensures the proper storage condition. Participates in stock
control programs (ordering, inventorying, and monitoring usage) as required. - Obtains emergency drug items or out of stock items as needed per pharmacy policy and reviews all non-formulary
requests for formulary alternatives, prior to dispensing. - Ensures correct medications are filled, dispensed and delivered appropriately.
- Defines problems, collect data, establish facts and draw valid conclusions, interpret technical instructions, and deal with
abstract variables. - Provides training and education to staff pharmacists, interns, and students regarding pharmaceutical care,
pharmacoeconomics, clinical pathways and clinical guidelines. Provides drug information and in-services to the
hospital’s professional staff as needed.
Education
Graduate of Pharmacy program
Preferences: Pharm D, with a completed residency in pharmacy practice
Experience
No experience required.
Preferred: 2 years hospital experience.
Certificates/Licenses/Registration
- Licensed Pharmacist in Florida
- Basic Life Support (BLS)
- Advanced Cardiovascular Life Support (ACLS)
Additional Information:
- BLS provided by the American Heart Association (AHA), required within 30 days of hire.
- ACLS provided by the American Heart Association (AHA), required within 6 months of hire - NOT required for PRN employees.
