Co-Occurring Case Manager
Position Title: Co-Occurring Case Manager
Position Type: Full Time; 40 Hours/Weekly

Department: Adult Clinical Services
Location: Ocean County
Requirements:
- Bachelor’s Degree in Human Services related field
- CADC Preferred, or CADC eligible/in progress
Responsibilities:
- Provide outreach and engagement services to community members experiencing issues related to substance use, substance use disorders and co-occurring mental health diagnoses
- Utilize Motivational and Educational Strategies to improve Social Determinants of Health
- Provide assertive case management to assigned caseload using Harm Reduction perspective and practices
- Document client interactions
- Coordinate care with multidisciplinary team
- Complete NJSAMS as needed
- Provide supportive counseling and crisis intervention services as needed
