HIRING AUTHORITY: Director/Health Officer
F.L.S.A. STATUS: Non-Exempt
EMPLOYMENT RELATIONSHIP: Just cause subject to a probationary period.
CURRENT HOURS AND STARTING TIME: Full-time position (37.5 hours per week). The starting time for this position is currently 8:30 a.m.
JOB SUMMARY:
Under direction, provide assistance to clients with navigation and enrollment for healthcare and related benefits; promote agency services and encourages participation in programs; promote healthy lifestyle behaviors and connect clients to resources; conduct outreaches in the community; perform related duties as assigned.
Provide guidance and assistance to clients in obtaining health and social services as needed.
Assess eligibility for a variety of programs, follow verification policies and procedures and provide enrollment assistance.
Provide basic education on health promotion and prevention behaviors.
Advocate for clients and make referrals as needed.
Provide services in a culturally appropriate manner.
May conduct home visits to assist with health education and to decrease barriers to services.
Conducts outreaches in the community as directed.
Maintain accurate records and assist in collection of data for reports.
Functions as a team member with other staff.
Performs emergency response roles as required.
Performs other duties as assigned.
Required Education and Experience:
Possession of a high school diploma or a certificate of successful completion of the General Educational Development Test- Successful completion of didactic education and training courses recognized by the Macomb County Health Department within one (1) year of hire
- Previous experience in public health or a related area
- Previous experience with MiBridges applications
- Ability to speak and understand a language other than English, including but not limited to Spanish.
- Possession of a valid Michigan driver's license and an operable insured automobile for authorized travel
Knowledge of:
Maternal/child health and social services systems and programs- Principles, practices, current developments, and techniques related to public health
- Standard office equipment and procedures
- HIPAA and other related laws and County policies
- Personal computer capabilities, operations and applications related to Desktop Operating Systems, MS-Office Suite (Word, Excel, PowerPoint), E-mail products and Web development products
- Organizational ability and good time management
- Basic skills in Word, Excel and Google products
- Perform data entry in multiple software applications
- Communicate highly complex information to people with varying levels of knowledge
- Work independently and meet deadlines with limited supervision
- Effectively speak, write and understand the English language
- Provide excellent customer services to internal and external customers via phone, email and in person with tact and courtesy
- Establish and maintain effective working relationships with staff, clients and the public.
- Maintain confidentiality of client information
- Read and understand laws, rulings and other published guidance
- Work independently
- Possess a valid Michigan driver’s license and an operable insured automobile for authorized travel
