Under direction of the Director of Community Outreach and Family Services or other supervisory official, the Passaic County Community Health Worker will have significant responsibility for educating the community on nutrition and wellness, and about the services and resources available in the community. Responsibilities include but aren’t limited to:

· Develops a health education program to address community health needs including wellness and nutrition.

· Identifies target populations and assesses nutrition education needs based on information from the New Jersey Department of Health and Senior Services, community health organizations, and other relevant health related data.

· Involves local community leaders in the planning, implementation, and maintenance of health education services and programs; collaborates with other agencies, and consults with subject matter specialists in the planning, implementation, and maintenance process.

· Applies appropriate evidenced based health education interventions to provide for the effective implementation of health education programs.

· Provides consultation and training to agency staff in the application of nutrition education techniques.

· Evaluates health education objectives.

· Provides guidance to the Department of Human Services to plan, organize, and develop a program of nutrition education for the community.

· Provides guidance and consultation to community organizations to develop and improve their health education activities.

· Assists individuals and groups to develop good personal health practices.

· Visits and speaks before members of health, civic, and welfare organizations.

· Creates and stimulates a public awareness of public health needs.

· Prepares written materials regarding the programs and functions of local clinics and the department.

· Provides advice to instructional and other school staff in the planning, development, and presentation of health education programs for students.

· Evaluates community health resources and health education programs.

· Prepares reports and correspondence. Maintains records and files.

· Creates dynamic and interactive presentations using a variety of media, that is tailored to the audience, and incorporates current research, best practices, and trends.

· Establishes and maintains cooperative working relationships with those interested in or involved in the work of the program.

· Provides education in schools and in the community for individuals, families, community, and community groups around issues concerning prevention, health, wellness, nutrition, and other topics identified as needs.

· Provides resources to callers seeking assistance.

· Attends and takes part in conferences, training institutes, and meetings concerned with programs and activities of the unit.

· Develops working relationships with involved agencies such as social services, Social Security, law enforcement, courts, probation and parole, child protection and permanency agency, clinics, hospitals, physicians, lawyers, clergy, schools, employers, and employment agencies.

· Flexibility in schedule is required. Occasional evenings and weekends are required.

· Other related duties as required by the department.

Requirements:

Education and Experience:

· Graduation from an accredited college or university with a minimum of a bachelor’s degree in Health Education, Community Education, Nursing, Public Health, Social Work or related field, CPS certification preferred.

· Two (2) years of experience assessing community health education needs and, in the planning, implementation, and evaluation of diversified health education programs in a local health department or state or local health agency.

· A minimum of three years’ experience working in a program or organization serving individuals with mental health and addictions concerns preferred.

Knowledge and Abilities:

· Strong written and verbal communication skills; the ideal candidate will be a persuasive and passionate communicator with excellent interpersonal and multidisciplinary project skills.

· Strong public speaking skills.

· Interest in the areas of prevention, wellness, nutrition, social justice, legal advocacy, prisoner reentry.

· Experience with individuals and groups related to case management, counseling, wellness, prevention.

Note:

NOTE: A Bachelor's degree in Nursing and a valid certification by the American Nurses Credentialing Center (ANCC) as a Community Health Nurse may be substituted for the Certified Health Education Specialist (CHES) requirement specified above.

Job Type: Full-time

Pay: $46,500.00 per year
