JOB SUMMARY:

The Peer Advocate fosters and builds relationships with peers in the community serving as the link between the individual and treatment.

• Provides peer support and motivation through continuous engagement, and by building rapport with patients.
• Assists the patient to identify treatment modalities appropriate for them, develop service plans, and help to identify barriers to treatment in order to promote successful linkage.
• Obtain and maintain WRAP certification for both individual and group sessions
• Report completion of WRAP plans to team leader and clinical supervisor
• Develop, facilitate, and maintain a WRAP group for clients
• Serve as a mentor to clients to promote hope, empowerment, wellness and recovery
• Help clients identify, understand, and combat stigma and discrimination associated with substance abuse, mental illness and develops strategies to reduce self-stigma
• Attends daily meetings to report on previous day’s visits and pertinent client information
• Provides peer counseling and support, drawing on common experiences as a peer, to validate clients’ experiences and to provide guidance and encouragement to clients to take responsibility and actively participate in their own recovery
• Submit productivity sheets daily to the administrative assistant
• Complete at least monthly chart updates to include filling notes, completing notes for visits, monthly summaries, yearly assessments, etc.

REQUIREMENTS:

• In active recovery for Mental Health, Substance abuse or Co-occurring issues
• Minimum of two years demonstrated sobriety/recovery
• A high school diploma or GED equivalent
• An active Maryland Driver’s License with two points or less.
• Maintain active CPR certification
• Excellent personal communication skills
• Ability to network and develop community relationships
