Description:
We are looking for a Certified Diabetes Care and Education Specialist with experience in women's health to join our interdisciplinary care team in delivering a patient-centered, memorable, care experience. In this role, you will provide diabetes care and education through 1:1 and group sessions, acting as the diabetes expert across all our practices, both in-clinic and remotely. Our ideal candidate will have a high level of empathy and strong communication skills.
What you'll do:
- Act as a Certified Diabetes Care and Education Specialist for those that are referred and/or interested in diabetes care and/or education:
- Conduct 1:1 in-person and/or virtual visits with our clients
- Message with clients about relevant diabetes care and wellness issues via our portal
- Connect clients to external referrals and/or resources as appropriate
- Act as the office expert on diabetes care and education, supporting staff training on diabetes prevention and care as well as relevant workflows.
- Lead relevant expert talks, classes, and support groups.
- Manage administrative activities such as:
- Respond to provider referrals and warm hand-offs
- Chart notes in our electronic health record (EHR)
- Collaborate with CNM & OB/GYNs on plans of care and areas of escalation
- Collaborate with LCSW to address any client mental health concerns
Qualifications & Experience:
- Certified Diabetes Care and Education Specialist - CDCES
- Experience and/or interest in working in women's health
- 2+ years of experience providing diabetes care and education
- Excellent communication, interpersonal, and organizational skills
- Strong computer skills and familiarity with EHRs
- General lifestyle medicine and/or wellness coaching certifications are a strong plus
