Family Nurse Practitioner - West Sacramento, California

Overview: Nexus HR is looking for a Family Nurse Practitioner in the West Sacramento, California area. The ideal Family Nurse Practitioner will provide diagnosis and appropriate treatment plans to patients.

About the Job
The Family Nurse Practitioner promotes and maintains patient health by providing medical services under the supervision of a physician.The Family Nurse Practitioner will provide both children and adults general and preventative care, perform check-ups, treat illnesses, request lab testing, and prescribe medicines.
Duties and Responsibilities:
- Conduct comprehensive patient assessments, including medical histories, physical exams, and diagnostic tests.
- Diagnose and treat acute and chronic illnesses and conditions.
- Develop and implement individualized treatment plans in collaboration with patients and other healthcare providers.
- Prescribe medications and therapies as appropriate, and monitor patients for adverse effects and drug interactions.
- Provide patient education on disease prevention, health promotion, and lifestyle modification.
- Order and interpret laboratory and diagnostic tests, and make referrals to specialists as necessary.
- Administer vaccinations and immunizations.
- Manage chronic diseases and conditions, including hypertension, diabetes, and asthma.
- Provide primary care services to patients of all ages, including children, adults, and older adults.
- Participate in quality improvement initiatives to improve patient outcomes and optimize healthcare delivery.
- Document patient care in electronic medical records and maintain accurate and up-to-date patient histories.
- Collaborate with other members of the healthcare team, including physicians, nurses, and support staff.
- Comply to ethical and legal standards of nursing practice.
Qualifications:
- 1-year experience as a Nurse Practitioner
- 1-year experience in Primary Care or comparable work experience.
- Active DEA license without restrictions.
- Must have a current California NP license.
- Must be able to provide telehealth and onsite services
- Able to work in Sacramento, CA.
- Experience with EMR and EHR platforms preferred.
