TEAM, Inc., a non-profit social service agency serving in 12 communities including the Valley (Ansonia, Derby, Oxford, Seymour, & Shelton), Milford, Waterbury, and Naugatuck has the following opportunity:

FAMILY SUPPORT SPECIALIST: Full-Time; 35 hours per week.

Position available to provide intake, assessment and case management for individuals and families. Duties will include 1. Complete intake applications (including eligibility determination) for clients using Agency central database. 2. Conduct client assessments to determine client needs and make appropriate internal and external referrals. 3. Act as a resource for Agency general assistance programs including housing assistance initiatives. 4. Provide case management to clients involved in Agency general assistance programs 5. Handle referrals from DSS to determine eligibility for programs. Provide assistance with DSS pre-applications as needed. 6. Follow-up with clients to determine services received and enter data into database. 7. Maintain accurate records through documentation and data entry into central databse in order to provide information for client tracking and outcome reporting. 8. Prepare reports as needed. 9. Develop relationships with community providers for purpose of conducting outreach for HUSKY program. Make presentations in the community regarding program eligibility requirements. 10. Handle telephone and walk-in inquiries regarding internal or external services. 11. Participate actively in appropriate local and state-wide meetings as well as internal staff and program meetings

Candidates must possess an Associate’s degree in Human Services or related field (Bachelor’s preferred) and at least one years experience working directly with a diverse population including case management. Other requirements include strong computer skills, knowledge of social services available in the area, good interpersonal and organizational skills, the ability to juggle multiple tasks and the ability to travel in the region. Bilingual a plus.
