Job description

• The Doctor’s Choice Home Health Agency is currently seeking experienced and dedicated Licensed Vocational Nurses to join our Home Health team in the Long Beach Area. This skilled nurse will be able to make visits within 10-30 miles distance from their home to the patient’s place of residence, in order to provide skilled care services as ordered by referring physician.

Per-Diem Status (Skilled Nurses are able to choose areas of their choice).

MUST be able to cover any or all of the following areas:

• Long Beach
• Signal Hills
• Cerritos
• Bell Flower
• Lakewood
• Paramount
• Belmont Shore
• Compton
• Seal Beach
• South LA
• Gardena
• West Carson
• Los Alamitos
• Lomita
• Cypress
• Torrance
• Downey

Minimum Requirements include:

• Graduate of a National League for Nursing accredited school of nursing
• Must have minimum 1 year of experience in nursing as an LVN
• Able to see patients at least 3 days a week (REQUIRED)
• Experience in EMR software (preferred but not required).
• Provide skilled nursing services/interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of skilled nursing interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
• Experience with Wound Care

Required Licenses and Certifications:

• LVN License in California
• Valid Driver’s License & Auto Insurance
• Current CPR certification
• Physical and TB from within the last 6 months

Pay: $43.00 - $50.00 per hour
