Follow-up Care Coordinator

Headquartered in Rockford, Tennessee, Remote Area Medical – RAM® is a nonprofit organization whose mission is to prevent pain and alleviate suffering by providing free quality healthcare to those in need. RAM accomplishes its mission by operating mobile clinics which offer a combination of dental, vision, and medical services to underserved and uninsured individuals, as well as disaster response and veterinary programs.

The Follow-up Care Coordinator reports to the Senior Clinic Coordinator & serves on the Operations Team.

Job Responsibilities

  • Build and maintain healthy strategic relationships with direct supervisor, personnel, volunteers and partners that would benefit department and organizational objectives
  • Maintain a constructive team unison with the Clinic and Volunteer Team to execute the mission of RAM by working closely with the assigned Clinic and Volunteer Coordinator Team members
  • Assist in mobilizing, recruiting, securing and serving Community Host Groups, volunteers and health care professionals; recording data as needed, building necessary reports as directed from clinic operations
  • Create one-pager reports within 48 hours of the completion of a clinic
  • Oversee review and audit of clinic records
  • Update one-pager reports once audit is completed
  • Connect patients with free or discounted care and treatments when abnormalities are discovered
  • Pull referral list within 48 hours of the completion of a clinic
  • Follow up on lab work from a clinic
  • Track and document follow-up care activities in RAM’s patient electronic health

record (EHR)

  • Recommend program opportunities, processes, procedures, policies, methods to improve relations, and functions of department objectives
  • Maintain familiarity with clinic event concepts, practices, and procedures; excel in relationship building
  • Rely on experience and judgment to plan and accomplish goals
  • Perform tasks and duties as directed by direct supervisor

Job Requirements

  • 3-5 years of experience in social work
  • Proficiency in Microsoft Office/365 suite of products as well as Google suite of products

Physical Requirements

  • Must be proficient in verbal, nonverbal, and written communication skills
  • Must be able to stand/sit for extended periods of time
  • Must be able to lift 20lbs.
  • Must have a valid driver’s license and be willing to drive RAM vehicles

Job Type: Full-time

Pay: $40,000.00 per year
