Always Compassionate Home Care is seeking a Field Nurse Supervisor (RN) for our White Plains location.

Responsibilities:

A. Full responsibilities for each case load assigned including:

• Initial patient evaluation and assessment.
• Mandatory and intermittent visits to patient homes.
• Contacting and assisting in the coordination of other necessary services.
• Manage coordination of care needs for patients when barriers or issues are identified.
• Oversee staffing coordination of cases to ensure that appropriate personnel are staffed to meet patient needs.
• Correspondence with Insurance company case managers regarding patient update as often as required.

B. Implementation and periodic review of nursing care plan and/or plan of care. This is to be done as often as indicated at least every 180 days, or as patient's condition warrants.

C. Assist in maintaining all clinical records, including:

• Review of patient records as requested.
• Notifying appropriate personnel if follow up on charting is needed.
• Representation at Quality Assurance meetings

D. Supervising, instructing and guiding nursing, auxiliary personnel and/or direct care personnel in providing high quality nursing services.

E. Home Health care staff member compliance with infection control and safety policies and procedures are monitored.

F. Assure that adequate and appropriate supplies and equipment are available and maintained at all times.

I. Cover weekend and weekday evening supervision as needed and make on-site visits of patients in emergency situations as needed.

J. Completing appropriate records regarding patient's condition

K. Keeping the agency informed at all times of patient's needs and care rendered.

L. Requirements of the state health department for the home health care agency are met through satisfactory NYS Dept of Health surveys.

M. Interviewing new field employees as requested.

N. Employee evaluations and updated physicals.

O. Inservice planning and participation.

P. Marketing upon request.

Qualifications:

• Registered Nurse with at least one year experience in Home Care or related Health Care field, or has graduated from a reputable Nursing Program and displays good leadership, critical thinking and assessing skills.
• Ability to work and deal with people.
• Ability to deal effectively with stress
