Job description
A trained, certified, para–professional, who is prepared to assist professional clinical providers (RN or therapist) in the care of the patient in the home and provides direct home and personal care services for the sick and/or disabled according to the plan of care. The duties of the Home Health Aide are planned and assigned by the professional clinical provider to meet the patient and caregiver's needs. The Home Health Aide is not a substitute for the professional clinical provider, but he/she may perform patient care under the direct supervision of the professional clinical provider.
ESSENTIAL DUTIES:
1. Performs all required responsibilities/duties addressing the ages and condition of the patients served.
2. Responsible for direct patient care which is determined, observed, supervised and documented on plan of care by a professional clinical provider.
3. Performs delegated tasks as authorized by the professional clinical provider depending upon situation and HHA experience/competence.
4. Performs routine household tasks as related for a safe, healthy and comfortable environment for the patient, as instructed and documented on plan of care by the professional clinical provider.
5. Documents patient status and care given, as reflected on required records.
6. Works as part of the Home Healthcare team participating in and identifying ways to improve the quality of care being provided to the patient.
7. Demonstrates respect and regard for the dignity of all patients, families, visitors and fellow employees to insure a professional, responsible and courteous environment.
8. Promotes effective working relations and works effectively as part of a department/program team inter and intra departmentally to facilitate the department's/program’s ability to meet its goals and objectives.
9. Attends all required safety training programs and can describe his/her responsibilities related to general safety, department/service safety, specific job–related hazards.
10. Follows the home and post-acute services division’s exposure control plans/bloodborne and airborne pathogens.
11. Complete all educational requirements as assigned.

6. Supports, cooperates with and demonstrates safe work practices and attitudes, follows safety rules – including universal precautions - reports and prevents/corrects unsafe conditions and behaviors, and participates in organizational and departmental safety programs.
7. Completes all required compliance standards that may be department specific and/or identified by the organization.
8. Maintains current licensure, registration and/or certification, as applicable, at all times.
STANDARD QUALIFICATIONS
- High School graduate or equivalent GED.
- 1 year of nurse aide or equivalent preferred.
- Must have current Certified Nursing Assistant Certification by the State of Michigan.
- Must have current BLS through the American Heart Association.
Other Qualifications:
- Good communication skills, which includes ability to read, write, speak, and understand the English language effectively
- Must demonstrate maturity and ability to deal effectively with issues related to delivery of services
- Must be able to arrange for transportation to patients’ homes
WORKING CONDITIONS:
A. Physical Effort:
- Strength
Position requires incumbent to constantly be able to push/pull and lift light objects, and occasionally be able to push/pull and lift objects less than 40 lbs.
- Manual Dexterity
Position requires incumbent to constantly perform simple motor skills such as standing and walking, also needs to occasionally perform moderately difficult manipulative skills such as typing.
- Coordination
Position requires incumbent to constantly perform gross body coordination such as walking, stooping, and tasks which require hand-eye coordination such as keyboard skills.
- Mobility
Position requires incumbent to constantly be able to walk, stand, sitting for prolonged periods, and occasionally stand for prolonged periods.
- Visual Discrimination
Position requires incumbent to constantly be able to see objects closely as in reading memos, charts, and other office paperwork.
- Hearing
Position requires incumbent to constantly be able to hear normal sounds with some background of noise as in answering phone, and occasionally be able to distinguish sound such as voice patterns.
- Mental Requirements:
o Concentration
Position requires incumbent to frequently concentrate on moderate detail with constant interruption, and be able to concentrate on fine detail with some or constant interruption.
o Attention Span
Position requires incumbent to frequently attend to tasks/functions for less than 10 minutes to over 60 minutes at a time.
o Conceptualization
Position requires incumbent to frequently understand and relate to specific ideas generally one at a time and several at a time, as well as understanding theories behind several related concepts.
o Memory
Position requires incumbent to frequently remember a task, or several tasks, at the beginning of a period for several days, as well as remembering tasks/assignments given to self and others over long periods of time.
B. Work Environment:
- Environment is typically in client homes on occasion may be in a healthcare facility.
- Must be able to drive; duties may include driving as part of the client care plan. Must have a valid state of Michigan driver’s license, along with automobile insurance.
- May be exposed to stress in the work place due to client condition and or family dynamics.
- May be exposed to extreme clutter and disorderly households.
