As a leader in the Specialty and Home Infusion space, InfuCare Rx has experienced tremendous growth year over year in the markets we serve. Rapid expansion has created a per diem opportunity for a Home Infusion Nurse. Positions are available throughout Pennsylvania for one-to-one home infusion nursing care visits.
Responsibilities and Duties:
- Responsible for always maintaining clinical competency via the demonstration of accountability, reliability, initiative, effective communication, technical skill, and competency of the clinical components of infusion nursing.
- Provide nursing care and treatment in the home in accordance with the physician’s plan of treatment and with company policies and procedures, and all applicable accreditation, federal, state, and local regulations.
- The Infusion nurse will apply appropriate ethical, legal, confidentiality and advocacy principles to the planning and delivery of patient care.
- The Infusion Nurse will provide professional nursing care and treatment of home infusion patients in accordance with the “Physician Plan of Treatment.”
- Obtain correct, appropriate, and timely physician orders for nursing care, collaborate with pharmacy staff to develop, and monitor goals of patient therapy.
- Promptly and efficiently communicate with the physician and other members of the patient care team as required discussing any changes to the patient status and or any recommendations.
- Additionally, the Infusion Nurse will coordinate patient admission with other members of the patient care team to meet all patients needs as assessed.
- Ensures patient competence and compliance with all self-care.
- Appropriately and timely document patient care activity, infusion services, and all communication with the patient care team that includes but is not limited to the patient, physician, physician’s office, etc.
- Ensure all documentation is legible, accurate, complete, and complies with company policy and procedure for electronic and or paper patient clinical record entries.
- Maintains client files per policies of the Pharmacy, in compliance with standards set by regulatory and accrediting agencies.
- Monitors patient progress to collaborative infusion goals to assess medical, psychosocial, and home care environment on an ongoing basis.
- Participate in call activities as directed.
- Troubleshoots problems regarding operational and clinical procedures.
- Responds to customer inquiries and resolves patient complaints.
- Participates in required continuing education and training as required for licensure regarding patient care and topics related to infusion therapy services as required by the State Board of Nursing and the company policy
- Assures proper maintenance of clinical records in compliance with local, state, and federal laws.
- Stays informed about changes in the field of nursing and home health care. Shares information with appropriate organization personnel.
- Serves and protects the Pharmacy by adhering to professional standards, company policies and procedures, federal, state, and local requirements, and URAC/TJC/ACHC standard of care.
- Perform other duties as assigned by supervisor.
Required Skills:
- Attention to Detail - Use excellent nursing skills to observe, interpret and monitor patient’s condition and notify appropriate personnel of any changes, acting as the patient advocate
- Interpersonal Skills - Working collaboratively with an exceptional team to help meet positive client care outcomes
- Communication - Discuss health care with patients and listen to their concerns, relay to the nursing team
- IV therapy - Must have exceptional IV skills
- Compliance - Assure proper maintenance of clinical records in compliance with local, state, and federal laws.
Required Education, Certification, Experience and Skill Requirements:
Successful candidates must meet the following requirements:
- Must be available Days, Evenings, Weekends.
- Must Have 5 years Hospital Experience.
- Current and unrestricted RN license
- At least 3 years’ experience as an RN, ideally with home care experience, must possess exceptional IV skills
- A Bachelor of Science in nursing, an associate degree in nursing, or have graduated from an accredited and approved nursing program
- Current CPR
- Proof of eligibility to work in the US
- Successfully pass annual competency checks
- Clean background and criminal record and must pass drug test
Physical Demands:
Required to stand, walk, sit, talk and hear; required to use hands to operate vehicles and office equipment; must be able to occasionally lift and/or move up to 25 lbs; specific vision requirements for this job include: close, distance and peripheral vision, and the ability to adjust focus, reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this position without compromising patient care.
