Responsible for providing services to Individuals andimplementing Individual interventions as required by the Individual SupportPlan. Complete necessary documentation in accordance with State and Programregulations, in addition to Maxim policies and procedures.

Essential Duties and Responsibilities:

• Ensure client’s Health, Wellness and Safety
• Encourage individual goals as they relate to learning andmaintaining independent living skills such as healthy lifestyle, personal care,and other activities of daily living.
• Crisis Prevention and Intervention
• Building and Maintaining safe and appropriate Friendshipsand Relationships
• Promote advocacy and individual rights
• Monitor and maintain detailed and compliant documentation onclient status, interactions, and behaviors as it relates to progress, concernsand other relevant details.
• Safe and appropriate implementation of behavior support planstrategies and interventions as it applies to the individual served.
• Accurate, timely reporting of suspected abuse, neglect, andexploitation.
• Utilize skills such as prompting, instruction, modeling, andreinforcement as it relates to person centered services delivered based on theIndividual Support Plan.
• Promote independence in activities of daily living,work/volunteer, learning, and leisure.
• Provide transportation, based on client needs, to affordcommunity integration, maintain medical support needs, and facilitate communityinclusion.
• Engagement in advancing client’s communication,decision-making, empowerment, and overcoming barriers to access necessaryservices, support and resources.
• Remain active in ongoing Education, Training and SelfDevelopment

Minimum Requirements:

• Ability to read and write proficiently in English
• Successful completion of the Direct Care Worker Competencyevaluation process
• Current CPR Card
• Current Health Screening (in accordance with stateregulations)
• Provide proof of one of the following qualifications:
• A valid and verifiable Pennsylvania nurses license OR
• A valid and verifiable Pennsylvania certified nurse aidecertification OR
• Certificate of completion from state approved home healthaide program. OR
• Has demonstrated competency by passing a competencyevaluation developed by the Home Care Agency which meets the requirements ofthe state OR
• Completion of a training program developed by a Home CareAgency or Home Care Registry, or other entity which meets the requirements ofthe state or training program meeting the standards imposed on the agency orregistry by virtue of the agency’s or registry’s participating as a provider ina Medicaid waiver or other publicly funded program providing home and communitybased services to qualifying consumers OR
• Successful completion of another program identified by theDepartment by subsequent publication in the Pennsylvania Bulletin or on theDepartments website
