Hospice Social Worker Job Requirements Include:

• Evaluate patients (existing, new and prospective) based on their needs, limitations and desires
• Address patient concerns and goals while maintaining constant communication with the patient as well as a case manager
• Build rapport with patients and their families
• Develop care plans for patients
• Plan, develop, organize, implement, evaluate, and direct services with a hospice interdisciplinary team
• Assist in the development, administering, and coordinating of department policies and procedure
• Works in collaboration with physicians and hospice team in patient evaluation and treatment, to further their understanding of significant social and emotional factors.
• Assists patients with adjustment to treatment, including problems of depression, anxiety, denial and/ or anger.
• Serves as patient advocate and liaison with physicians, families, and community to ensure continuity of care.
• Documents history, assessments, and interventions to reflect a comprehensive and integrated approach to patient care

Licensed Clinical Social Worker Job Requirements Include:

• MSW required
• Excellent communication & interpersonal skills
• Highly Organized, Professional & Motivated.
• Strong problem-solving skills with ability to multitask & prioritize
