Job Summary

To support the work of the Litchfield County Coordinated Access Network (CAN), The Gathering Place, and the residents of the Greater Torrington Area to engage any household in need coming through the CAN with the appropriate level of intervention needed to support their return to safe and stable housing in the community. This position provides dedicated case management services to individuals and families and continuously engages clients in developing and realizing a plan to end their housing instability through diversion, shelter placement, rapid rehousing, and other interventions. Targeting decentralized, community-based supports to folks at the moment of crisis will allow for fewer entries into homelessness through more exhaustive prevention/diversion efforts, and shorter durations of homelessness. The Navigator will bridge the gap between the time of crisis, entry into the CAN system, and the return to permanent housing in the community.

Job Responsibilities

  • Support triaging households who are experiencing homelessness or meet Housing and Urban Development's (HUD) definition of at-risk to connect with the appropriate resources related to housing security and stability. Assist clients in connecting to 211.
  • Proactively engage all households in diversion interviews and assessments and assist clients with meeting basic needs, including securing reliable means of contact, personal identification and documentation, income verification, homelessness verification, and disability verification and upload them into Homeless Management Information System (HMIS).
  • Engage households accessing the Coordinated Access Network (CAN) who are not currently receiving housing.
  • Proactively identify opportunities for CAN and local service providers community education.
  • Attend meetings as necessary to inform key stakeholders and public officials of housing trends and barriers. Maintain organized files and adhere to timely data entry into HMIS, SmartSheet, and/or other data system.
  • Proactively form linkages with community providers, including emergency shelter, transitional housing and domestic violence shelters that serve the targeted population, security deposit providers, landlords, and others.
  • Participate in trainings related to best practices and CAN system improvement- including but not limited to diversion, motivational interviewing, trauma-informed care, HMIS, domestic violence, etc. to ensure consistency across CAN and improved service delivery.
  • Other Relevant duties as assigned.

Qualifications

  • High School diploma required
  • College degree preferred
  • Minimum five years of experience providing homeless services, and or shared lived experience
  • Excellent written and verbal communication skills, including telephone communication
