JOB SUMMARY/ACCOUNTIBILITY:
The Resource Navigator facilitates the coordination of community services rendered to clients; provision of concrete services; collaboration with team members and community partners; and advocacy tasks, such as assisting in making appointments and securing financial and property tax benefits. The Housing Navigator will have strong engagement and motivating skills. The housing navigator links families with housing and resources.
GENERAL RESPONSIBILITIES:
  • Become active in the Landlord/property management community to identify housing opportunities for households.
  • Regular coordination with Program managers and Housing specialist, including attending collaborative working group meetings.
  • Conduct comprehensive housing assessments, housing planning and client-driven housing searches based on the individual clients' needs.
  • Act as a client advocate and assist in Develop Housing Stability Plans and budgets
  • Collect all necessary documentation needed to support assistance.
  • Extensive record keeping, data entry and documentation of services in Client Management systems and databases
  • Maintain comprehensive files and statistics required by funding sources.
  • Keep current with landlord laws, ordinances and regulations that impact housing. Attend trainings, workshops, seminars, community events and conferences related to this position.
  • Eligibility - determine client eligibility for services; gather information from all relevant sources; develop a person-centered plan for service and intervention based on client needs and choices.
  • Assess/Reassess – conduct initial and follow up in-depth assessment of client’s overall status.
  • Linking/Coordinating – identify resources needed by the client, determine client eligibility, procure vocational and support services based on client needs and choices, assist client in applying for services, maintain a working knowledge of community programs pertinent to the client population and apprize administrative staff of unmet community needs.
  • Case Documentation – record progress notes and statistical information detailing pertinent activities and services.
  • Monitoring – on-going face to face contact with clients to assure the effective provision of services and monitoring clients’ progress toward treatment goals and objectives.
  • Perform accurate and timely completion of clinical and statistical records of staff interventions and activities.
  • Assists in Coordination of group learning classes, including financial literacy, as needed by the program, and directed by administration
  • Conduct activities in support of the team to provide program consistency.
  • Effectively maintain files.
  • Develops outreach programs and strategies to bring awareness of Housing Resources and our services
  • Serve as the primary point of contact for landlords concerns and complaints, acting as a liaison to connect landlords, tenants and the housing case managers for those in the program.
  • Educate households in their search for housing such as identifying housing, filling out rental applications, interpreting leases and understanding tenant right and responsibilities. Work with clients on a one-on-one basis to provide education, guidance and referrals tailored to each client’s unique circumstances.
  • Create and/or maintain a database of landlords and properties and the relevant selection criteria they utilize. Communicate updates to relevant staff and community partners
  • Perform other duties as required.

POSITION QUALIFICATIONS:
  • AA/AS/ASW and 2 years experience in human services preferred
  • Experience working with clients in challenging personal financial situations, including histories of incarceration, substance abuse, domestic violence and homelessness
  • Ability to handle multiple tasks
  • Familiarity with affordable housing issues and Housing First approach
  • Familiarity with property assistance programs.
  • Proven ability to work well with others, including community leaders, elected officials, a diverse collection of team members, and people experiencing extreme poverty
  • Experience in conducting group training and working with diverse populations.
  • Must have valid driver’s license and transportation to conduct job related travel
  • Experience working with households experiencing homelessness strongly preferred

WORKING AND ENVIRONMENTAL CONDITIONS:
Works in a normal office environment. Because of multiple sites within the agency and the need to meet with others within the community, it is common to drive to several locations, sometimes in inclement weather
