Bureau: Family Health Services / Adult Evaluation and Review Services (AERS)

Full-time County Contractual

Hours: 40 hours per week

Salary: $32.57 per hour

Job Description

Nature of Work:

Perform inter-rai assessments in community settings for a variety of programs that serve people with disabilities and seniors. Schedule with clients, their caregivers, family members and/or interested service providers (when appropriate) to meet and complete the applicable evaluation/assessment. Strive for accuracy and timeliness in assessment and referral. Communication regarding the worker’s recommendations with client, caregivers and providers. Meet and discuss cases with the multi-disciplinary team. Enter and maintain correct information in the Long Term Care Services and Supports (LTSS) system, Advanced Information Systems Management (AIM). Complete all ancillary documentation and follow-up, as well as submit billing sheets when necessary. Maintain adequate productivity, with a goal of 25 completed considered excellent in a month. Maintain Social Worker license.


Minimum Qualifications

Education: Master’s degree in social work from an accredited college or university.

Experience:

Long-term care, hospital/medical setting, home visiting, home health, Adult Protective Services, community-based mental health services



License or Certification:

LCSW-C, LMSW



Desired or Preferred Qualifications:

Community-based health care, LCSW-C
