We are Now Hiring a Incident Management Coordinator in Nashville, TN .The Incident Management Coordinator is responsible for monitoring the systematic operations of incident management and internal investigations of residential and Employment programs for the developmentally disabled population.


Requirements:

  • Bachelor’s degree in a related field or equivalent, and three years related experience
  • Approved DIDD investigator preferred
  • Any equivalent combination of experience and training
  • Valid driver’s license with satisfactory driving record
  • Commitment to and knowledge of community-based support for persons with disabilities
  • Working knowledge of management of an effective QA program, including training, monitoring, conducting and documenting investigations, addressing violations and monitoring corrective actions.
  • Ability to maintain effective relationships with other management staff, employees, regulatory/oversight agencies and the general public.
  • Excellent time management skills and the ability to develop and implement multiple priorities.
  • Ability to present facts and recommendations effectively in oral and written form.
  • Independent judgment is required to plan, prioritize, and organize diversified workload.
  • Ability to demonstrate poise, tact, and diplomacy.
  • Excellent presentation and verbal skills.
  • Experience using Microsoft Office 365.
  • Knowledge and understanding of local regulatory agency operations.

Responsibilities:

  • Reviews all incident reports to verify they are completed thoroughly with all required elements and accurately describes the incident and submitted as required and in a timely manner as required by the funding source.
  • Monitors Therap’s General Event Records to verify incidents are properly documented and reported.
  • Follows up to verify that all required notifications have been made to the individual’s team, management, clinical staff and regulatory agencies.
  • Conducts weekly incident management meetings and maintains incident management database as required. Enters all incident information into database timely.
  • Reviews investigation recommendations with the QA/Incident Management Committee and responds to reports in a timely manner.
  • Initiates and Monitors the implementation of all QA/Incident Management Committee recommendations and reports any issues.
  • When investigations are conducted internally: Verifies that investigations are initiated timely. Acts as the point of contact for investigations and coordinates the investigation process, collecting information requested by the investigator, scheduling staff interviews and visits to the homes/programs.
  • As needed and allowed by the funding source, may conduct internal investigations.
  • Responds timely to state investigations per regulations.
  • Conducts and incident/ investigation follow up as required or requested by the QA/Incident Management Committee.
  • Monitors the agency’s compliance with incident management and abuse and neglect policy.
  • Conducts root cause analysis/analysis of causal factors to prevent future incidents of a similar nature.
  • Conducts quarterly and annual trend analysis studies as required and presents reports to management and the national team for review.
  • Member of the National & Local QA/Incident management committees
  • Chairs local Health & Safety Committee meetings
  • Conducts staff training as needed
  • Cooperate with internal and State inspections and investigations
  • Attends staff meetings as determined by supervisor
  • Attends conferences, seminars and training relevant to the position
  • Maintains a master file of all audits
  • Other duties as assigned by the Regional/State/ Executive Director

  • While performing the duties of the job, employee travels by automobile, utilizing own vehicle, and is exposed to changing weather conditions
