Position Summary:

The Villa Marie Claire Intake Associate is primarily responsible for obtaining the referrals necessary for the initiation of hospice services at VMC and their successful reimbursement. The Intake Associate ensures all medical record documentation and processes are completed within the patient's medical records. The hospice intake associate works with the hospice team to identify appropriate patients for VMC admission and/or coordinates with other members of Holy Name Hospice to facilitate hospice admissions in the community (home or another facility.)

Requirements:

Education: Bachelor’s Degree preferred

Skills: Google Suite products, including Google Docs, Sheets, etc. Ability to manipulate CV/Excel reports and use all Excel/Google Sheet features. Ability to navigate electronic medical records. Understanding clinical coding / ICD10, as well as deductible, co-insurance, co-pay, OOP max, navigating procedure codes, and prior authorizations processes for all insurance carriers

Experience: 2 plus years of experience in hospice or related services. Experience in discharge planning and hospice reimbursement, some finance background would be helpful.

Certification/License: BLS by American Heart Association. Valid driver’s license and means of transportation.

Essential Functions of the Job:

  • Evaluates patients referred for Hospice care to determine appropriateness for admission, evaluation of level of care, insurance benefit eligibility, and refers patients needing other sources of care to appropriate services
  • Interprets inpatient VMC Hospice and Home Hospice policies to patients, hospital staff, and other referral sources
  • Consults with physicians, social workers, nurses, and other referral sources to establish a coordinated admission plan including transportation
  • Communicates daily with medical staff patient’s current condition, level of care, obtains ongoing need for insurance recertification, and provides the information to VMC clinical and billing staff
  • Participates in educational activities, interdepartmental meetings and committees
  • Assists in the verification of insurance coverage and provides the information to hospice and billing teams.
  • Adheres to all medical record analysis procedures to ensure a complete medical record.
  • Electronically analyzes scanned telephone orders for appropriate physician e-signature.
  • Ensures all face to face visits performed by my Medical Director are stubbed into the patient's medical record.
  • Accurately prepares and scans medical records in response to RAC and other payor requestors.
  • Accurately sorts and files loose papers into correct medical records.
  • Accurately prepares medical records for outside storage.
  • Provides assistance to the Director with medical record projects in the department.
  • Answers the telephone in a clear and concise manner, provides information in a courteous and professional manner.
  • Accepts other medical record duties as assigned by the supervisor in a professional manner.
  • Will drive from our corporate office in Hackensack, NJ to the hospital in Teaneck, NJ as needed to retreat documentation.
